    Practicing everyday tasks in a safe space is key to recovery. Our residents and patients make beds in a simulated apartment, cook in a functioning kitchen and get behind a steering wheel again.

    Smart home strategies

    Patients and their caregivers learn how to avoid return trips to the hospital by recognizing heart failure and taking the right steps, right away.

  • Home follow-up

    Home follow-up

    Continuity is key. Our team calls every patient 72 hours after discharge, and again a week later.
